Who is eligible for services?

Any adult patient with transportation and mobility limitations is eligible for Physican’s Home Visits. We are committed to delivering personalized quality medical services to all of our patients in the safety and comfort of their homes, emphasizing wellness and the management of chronic conditions including: diabetes, hypertension, congestive heart failure and other medical issues.

How are my services paid?

Our home medical visits are billed in the same way your doctor’s office visit is billed. We accept Medicare, Medicaid and secondary insurers. Our billing specialist can help you with your individual questions.

You will not be asked for payment at the time of your visit. Physicians Home Visits are covered by Medicare and secondary insurers. We will bill your insurance carrier on your behalf.
